Self Enrollment Process

If you have decided to become a member of the Consumer Health Choice Association and have also chosen an Intelicare Program, please be ready to fill-in the information regarding the primary applicant, any spouse and/or dependent names and date of births, beneficiaries, payment information, and finally to review membership terms & conditions.

You are becoming a member of the Consumer Health Choice Association (CHCA) where a one-time processing fee and monthly association fee applies.

*Patriot Health Florida, Inc. Program Features are not available in CA, CT, FL, ID, KS, ME, MT, NH, NV, OK, VT and WA.  

Exceptions: 4-Tier Pharmacy feature is not considered a discount medical plan and is available in FL, is also available in ID and ME but is not available in CA, CT, KS, MT, NH, NV, OK, TN, VT and WA.

24-Hour Nurse and Counseling Hotlines are service programs available to all CHCA Members of Intelicare Programs.
